Coordination is an act of care

Your Care Coordinators is an NDIS support coordination service. We walk alongside participants and their families — making sense of plans, connecting the right supports, and building the confidence to navigate the system independently.

Our mark is a heart drawn in two open strokes cradling a supporting hand. Two parties, one shared purpose, a hand that holds without gripping. That is the whole idea, and it is how we try to work: with you, not for you.

We work in person and by telehealth, so where you live does not decide the quality of coordination you get.

What you can expect

  • A reply within one business day, and a person who knows your plan.
  • Options laid out honestly, with the trade-offs named.

Vanessa Sinclair

Senior Support Coordinator

Vanessa has an array of knowledge and vast experience, having worked as a Support Coordinator in the disability sector for over 9 years. Specialising in supporting people with physical and psychosocial disability, she is dedicated to empowering independence and personal growth.

With a person-centred approach, Vanessa believes in respecting the individuality of every person she supports — creating solutions that help reach specific, personal goals and objectives.

In practice that means starting with listening, being straight about what the system can and cannot do, doing the chasing so you do not have to, and gradually handing over the knowledge so you can drive more of it yourself.
