Level 2

Support Coordination

The everyday work of turning funding on paper into supports that are actually running in your life. We go through your plan together, work out what matters most, and put the right services in place.

A plan tells you what has been funded. It does not tell you which provider has capacity this month, what a fair price looks like, or what to do when someone does not turn up. That gap is the job.

Who it’s for: Participants with Support Coordination funded in their Capacity Building budget.

Level 3

Specialist Support Coordination

For high-complexity situations — where health, housing, justice or risk issues are tangled together and progress needs an experienced practitioner steering a whole team.

The work is more intensive: mapping risk, holding services to their part of the plan, and reducing barriers so the ordinary supports can finally do their job.

Who it’s for: Participants with Specialist Support Coordination funded in their plan. If we think you need it and it is not funded, we will say so and help you evidence it.

Recovery

Psychosocial Recovery Coaching

Recovery-oriented support for people living with psychosocial disability, built on the understanding that recovery is not linear and good days and hard days both count.

We work at your pace on the things you name — routine, connection, confidence, work, study — and keep supports steady through the harder stretches.

Plan Reviews & Reports

Reviews go better when the evidence has been building all year. We keep track of what has worked, what has not, and what it cost — so the report writes itself from real life.

Then we prepare with you, so you walk in knowing exactly what you are asking for and why.

How it is funded

Where support coordination sits in your plan

Capacity Building

Support coordination is funded under Capacity Building — Support Coordination. It is a separate line from your Core supports.

Set hours, not open-ended

Your plan funds a set number of hours. We track them with you so there are no surprises late in the plan year.
